With the authority given by the GCG to fill-up the vacant Second Level positions as listed on the List of Vacant Positions link below, interested applicants who meet the qualification requirements, must submit the following pertinent documents:

  1. Duly accomplished Personal Data Sheet (CSC Form 212 – Revised 2005), which may be downloaded from the CSC Website – i.e.,;
  2. *Transcript of Records / Diploma
    • Applicants who have post-graduate studies must submit certified true copy of Official Transcript of Records with Special Order if completed;
  3. *Certificate of the appropriate / required eligibility / license / rank; conferment; i.e.:
    • Career Service Eligibility
    • Career Executive Service Officer (CESO) Rank appointment or CES Eligibility Certificate
    • BAR or Board Rating
    • Others
  4. *Certificate/s of honors / commendations / citations received;
  5. *Certificate/s of Seminars or Training attended;
  6. Strategic Performance Management System (SPMS) for the last two (2) rating periods (for current government employees only)
    • Employees with Outstanding rating must be duly justified by their respective Managers.


* If photocopies are submitted, the documents must be certified as “true copy” by the authorized Records Officer or Custodian and Alternates.

All applicants should submit their letter of intent, addressed to the Chairman, Personnel Selection Board (PSB) thru the PSB Secretariat, indicating the desired position/s (limited to 3 positions), arranged according to preference, together with the documentary requirements to the Personnel Division within ten (10) calendar days from the date of posting hereof.

Late submission of applications and supplementary documents shall no longer be accepted by the Personnel Division. However, applicants may request for reconsideration in writing addressed to the Chairperson, PSB.
